60 (1) For the purposes of subsection 104.01(2) of the Act information may be made available to a person or body pursuant to a written request by an individual or their representative if the request is signed within one year before the day on which it is received by the Minister.

(2) The person or body may, on application, have annual access to the information if the request referred to in subsection (1) indicates that the individual or representative consented to the provision of the information on an annual basis, and until the individual or representative revokes the request or the individual dies.
