Schedule V – Annex I: Chapter V – Civil Defence, Article 14 – Identity Card

Front Side View: On the front side view of the identity card there are two blue triangles on an orange ground. The two triangles are in the top two corners. Between the two triangles is the phrase "space reserved for the name of the country and authority issuing the card". Below that phrase is the title, "IDENTITY CARD for civil defence personnel". Below the title, the front view of the identity card also includes a space for the following information: the individual's name, the individual's date of birth or age, identity number, the date of issue, the card number, the date of expiry, and the signature of the issuing authority. In the middle of the front view of the identity card is the following paragraph: "The holder of this card is protected by the Geneva Conventions of 12 August 1949 and by the Protocol Additional to the Geneva Conventions of 12 August 1949, and relating to the Protection of Victims of International Armed Conflicts (Protocol I) in his capacity as..." Following this paragraph is a space to write the individual's capacity.

Reverse Side View: On the reverse side view there are 7 boxes. The boxes provide space for the individual to write his/her height, eyes, hair, other distinguishing marks or information, his or her weapons, as well as a space to provide a photograph, a stamp, and the signature/thumbprint of the cardholder.
